We accept cash, personal check, Square and PayPal and/or we can bill your insurance company. Charges not covered by your insurance such as deductibles, co pays and balances for non-covered services are your responsibility. Payment is due at the time of service.

Cancellation Policy

As a courtesy to other patients, we ask that you give 24-hour notice for cancellations.

We strictly adhere to charging for missed appointments with less than 24-hour notice. We reserve the right to charge for the missed appointment at the rate of a normal office visit, which is approximately $100. This fee cannot be billed to your insurance company.

In the event that you are unable to provide sufficient notice of cancellation as outlined above, we will require your pre-authorized debit/credit card information for any subsequent appointment scheduling. A subsequent missed appointment will result in a charge of the respective amount to your debit/credit card.
